VUMC Launches Collaborative Initiative to Enhance High Blood Pressure Management

Vanderbilt University Medical Center, funded by PCORI, will adopt a team-based approach to manage hypertension. With nearly 50% of U.S. adults affected, this initiative aims to improve blood pressure control through pharmacist-led telehealth visits, benefiting over 1,600 patients.
